Complaint and Reporting Forms

Please carefully read the descriptions for each complaint type below and select the one that best fits your needs for reporting an issue to Central State University. 

  • Please use this below should you find yourself in conflict with a University process or policy—academic or non-academic—or if you are dissatisfied with the delivery of services provided by a University faculty or staff member.

    This form is NOT for issues related

    • to grades,
    • the Americans with Disabilities Act,
    • or discrimination, harassment, sexual misconduct, or retaliation under Title IX.

    Issues related to grades follow the Grade Appeal Process outlined in the current University catalog, and issues related to the Americans with Disabilities Act, or discrimination, harassment, sexual misconduct, or retaliation complaints should be reported using the process outlined in the Student Handbook.

  • Central State University encourages victims of sexual misconduct, discrimination, and harassment to talk to somebody about what happened so they can get the support they need and so that CSU can respond appropriately. Though CSU will keep reports as confidential as possible, the University cannot guarantee the confidentiality of every report or complaint.
