FMLA Certification for Employee – This form must be completed and submitted to the Human Resources Department in order to apply for Family and Medical Leave to take care of yourself during a medical emergency, childbirth, or adoption.

FMLA Certification for a Family Member – This form must be completed and submitted to the Human Resources Department in order to apply for Family and Medical Leave to take care of a qualifying family member during a medical emergency, childbirth, or adoption.
