ALL BAND STUDENTS ARE REQUIRED TO:

1. Submit a student information form prior to entering band camp.   Click to download the Student Information Form.
2. Submit a completed physical examination and immunization form prior to entering band camp. Click to download the Student Health Record.
3. Submit an Anti-Hazing Form prior to entering band camp. Click to download the Anti-Hazing Form.
