LaShaunta Jones, ATC/LAT serves as the Head Athletic Trainer for Central State University. She is a graduate from Wright State University with a degree in Athletic Training. She oversees the daily operations of the Marauder Training Rooms and supervises the treatment and care of the Marauder student-athletes.

Dear Parents and Student-Athletes:

On behalf of the Department of Athletics, I would like to welcome you or welcome you back for another year of Marauder Athletics. We are looking forward to the upcoming seasons.

The purpose of this letter is to inform our student-athletes of the medical procedures that must be completed prior to participating in athletic competition at Central State University. All incoming freshman and transfers must complete a Pre-participation Examination prior to coming to CSU.

The pre-participation examination form is enclosed with this letter. Please have this form completed and signed by your family doctor, clinical doctor, or orthopeadic doctor. An address and phone number of the examining doctor must also be provided on the form.

The insurance and emergency contact form must be completed by the parent/guardian of the student-athlete. Please include a front/back copy of the student-athlete’s insurance card and prescription cards. You will find this document enclosed with the packet as well.

All other medical documents, explanation letters, and insurance policy/procedure forms can be found on the university website at www.centralstate.edu under the athletics link. Please be advised that your son/daughter will not be allowed to participate in Central State University Athletics without the proper paperwork on file prior to participation. The Athletic Department will assume no responsibility for injuries, medical bills, or medically treat any student-athlete who does not have paperwork or a pre-participation exam on file.
